Payment / Insurance Info

Clients who choose to use insurance for services are responsible for knowing their plan, benefits, visit limits if applicable and co-pays. This information is available by calling the number on the back of your insurance card.

Dr. Maurer and Dr. Warholic accept Blue Cross Blue Shield, Blue Care Network, Priority Health, Aetna, ASR Health Plans, and self (private) pay.

We can accept payment via credit card or health savings account (HSA).

Whether using insurance or private pay, you are required to keep a current credit card on file.

Most evaluations cost about $2,500, although specific fees will be discussed as part of the consultation and intake process, as these can vary based on your child’s assessment needs. A Good Faith Estimate will be provided after intake which includes CPT codes and cost estimates for the appointments to follow.

Testing/Evaluation No Show/Late Cancellation Policy

No show/Late Cancellations: A "late cancellation" is when an appointment is cancelled with less than 24-hour notification to the front office during hours of operation so that there is an opportunity to rebook the clinician’s time. Exceptions for late cancellation are illness and significant family emergencies. A "no show" is when the client has not arrived by 15 minutes after the start time of the appointment (in-person or virtual). Please call the office if there is something unexpected preventing on-time arrival. We will do our best to accommodate but there will be some circumstances where the appointment cannot occur because of the lack of time. No show and cancellation fees are billed directly to the client and are based on the private pay rate as this is not billed to or covered by insurance.

For the initial intake appointment: 1st missed appointment fee = No charge

2nd missed appointment fee = full session cost.

We will not offer a third reschedule.

For the testing appointment (usually several hours long): if the appointment is missed, the charge is half of the fee per hour of scheduled testing. We will offer two reschedule opportunities, not a third.

For the feedback appointment: Missed appointment results in full session cost. We will offer two reschedule opportunities, not a third. We would send the completed report since feedback was not possible.
